Healthcare for the Homeless Program

The LifeSpring federally qualified health center program carries a special designation, known as “Healthcare for the Homeless.”  This allows us to provide comprehensive primary care services to homeless patients, those unstably housed, and those at risk of homelessness, at low or no cost.  Our healthcare for the homeless program ensures that no patients are without the necessary primary care and patient centered medical home support needed to lead a healthy life.  Call us at 812-280-6606 for more information.

Permanent Supportive Housing Program

LifeSpring’s Permanent Supportive Housing Program is a HUD funded program that provides housing to individuals and families who are literally homeless and have a documented disability. Each household has a case manager that works with them to meet the needs that are identified through an assessment. These needs may include financial management, employment, education, child care, mental health services, substance abuse services, medical care and daily living skills. The goals of the program are to maintain housing, increase income, and work toward self-sufficiency. For more information, please contact the Program Manager at (812) 981-2594, extension 2023.
